Please follow these directions for instrument repairs:

1. Clean and sterilize the instruments to be repaired

All instruments / equipment MUST be sterilized and shipped in a sterile pouch with an indicator reading of sterility, in accordance with OSHA Standards.

2. Download and fill out your Repair Form, clearly stating:

This completed paper form must be sent in the same package as your instruments to be serviced

3. Use a sturdy shipping box with sufficient packing material to protect your instruments

Completed REPAIR FORM must be inside the box with your instruments
